Consent forms
The NHS has asked that schools distribute their consent forms to families as soon as they are received.
All schools should now have received their forms. These are sent to school offices requiring a signature so please check with your office staff if a delivery has been received. Any schools yet to receive the forms should contact the NHS team (0121 466 7635 or BCHC.covidschools@nhs.net)
